At 360 Concussion Care, we’re here to support you with compassionate, evidence-based care. Our team of doctors and therapists specializes in concussion management and works together to help you safely return to school, work, sports, and daily life. Founded by internationally respected clinician-researchers, we’re also committed to advancing care through research that improves outcomes for all.
Your First Visit
Your visit starts with one of our Research Assistants, who will gather some background information and go over a few quick assessments, including a couple of questionnaires and blood pressure readings. They’ll also tell you about optional research studies you can take part in—these help us learn more and improve care for others.
After that, you’ll meet with both a physician and a rehabilitation specialist. The physician will confirm your diagnosis and outline a care plan, while the rehab specialist will help you with symptom management and strategies to get back to what’s most important to you. Please allow 1.5 to 2 hours for your appointment, and arrive 10 minutes early.
Before Your Appointment
You’ll receive an email with a link to complete some brief pre-visit questionnaires (about 15–20 minutes). These are really important—they help our team prepare for your visit and make the most of your time with us. If they’re not completed ahead of time, we may need to reschedule your appointment.
Staying Connected
Once you’re registered, you’ll get access to our secure Patient Portal. Through the portal, you can message your care team, view documents, and manage appointments. It’s the best way to stay up to date.
Forms & Letters
Need a letter for school, work, or sports? Let us know in advance by noting it in your registration questionnaire. We’ll include return-to-school or work plans in your initial visit if needed. Please note: forms and letters are not covered by OHIP and may come with an added fee.
Cancellation Policy
If you need to cancel or reschedule, we ask for at least 48 hours’ notice (two business days). Changes made within 48 hours will be charged 50% of the appointment fee (+ HST), and cancellations within 24 hours or no-shows will be charged in full. These fees are not covered by insurance, even for OHIP visits.
