How it works
Here's what to expect.

01
Get referred
Most patients need a referral from a physician or nurse practitioner.

02
Get assessed
Your first visit includes a physician assessment and a rehab specialist evaluation. You'll leave with a clear picture of where you stand.

03
Get better
Your care team builds a treatment plan around your symptoms, your schedule, and your goals. You won't be passed around. Everyone works together.
Our services
Everything you need, in one place.
We bring together the right specialists so you don't have to coordinate care on your own.
Physician Care
Your assessment starts here. A concussion physician reviews your symptoms and leads your care plan.
Physiotherapy
Hands-on treatment for headaches, neck pain, dizziness, and physical symptoms from concussion.
Occupational Therapy
Help getting back to school, work, and daily life when symptoms are getting in the way.
Massage Therapy
Targeted treatment for muscle tension, headaches, and pain that often follows a concussion.
Mental Health Support
Anxiety, low mood, and sleep problems are common after concussion. We have support for that too.
Neuropsychological Testing
Detailed cognitive assessment for memory, attention, and processing. For when you need clear answers.
Buffalo Bike and Treadmill Test
An exercise-based assessment that tells us exactly how activity affects your symptoms.
PREVENT Program
Neuromuscular training to reduce your risk of re-injury after you return to sport.
Coverage
Wondering what's covered?
Your MD visit is covered by OHIP. Most patients also have a second assessment on the same day with a rehab specialist. That part has a fee.
If you have WSIB, motor vehicle accident (MVA), or extended health coverage, we direct bill. Contact us before your first visit and we'll sort out what applies to you.

Concussion prevention
The PREVENT Program.
Getting cleared is one thing. Staying healthy is another. Returning to contact sport after a concussion puts you at higher risk for a second one. The PREVENT program is our neuromuscular training program designed to reduce that risk.
The program runs after you are medically cleared and is built for athletes returning to high-contact play. It trains the movement patterns and physical control that break down under fatigue.
For athletes
Athletes returning to contact sport after receiving medical clearance for a concussion.
What it involves
Supervised neuromuscular training sessions focused on balance, reaction time, and movement control under physical stress.
How to enroll
Ask your physician or care team at your clearance appointment. [PLACEHOLDER: confirm enrollment details and session structure]
Why choose 360
Why patients choose 360.
At 360, every concussion assessment starts with a physician. You see a doctor first, someone who can look at the full picture, not just check boxes.
The concussion treatment protocols we use are grounded in current research. Neuropsychology, physiotherapy, and medicine work together on your care. If you are working toward return to sport clearance, that process is coordinated, not left to you to piece together across separate referrals.

Patient stories
What patients say.
Dr. Zemek and I have collaborated to advance concussion awareness through Rowan’s Law. He’s a visionary leader who understands the impact of concussion on patients and their loved ones.
Eric Lindros
NHL Athlete
"After my son's second concussion, I didn't know who to call. 360 actually explained what was going on and what the timeline looked like. By the time he was cleared to play, I felt confident about it."
Jacob S.
Parent of athlete
As a parent of a child who has had concussions, I am extremely grateful for the work Dr Zemek have done to advance concussion care. He seeks to answer the questions that matter most to patients and families. He makes a difference because he engages with families as partners to help guide his research
Kathy Leeder
