Part 1
MD assessment

A concussion physician reviews your history, symptoms, and current status. They conduct a comprehensive physical exam, makes a diagnosis and develops a management plan. This visit is covered by OHIP.

Part 2
Rehab specialist evaluation

An occupational therapist, physiotherapist, or physiotherapy assistant completes a functional assessment on the same day. This part has a fee.

What happens after your first visit

If treatment is recommended, your care team puts together a plan around your specific symptoms and goals. Some patients need a few visits. Others need more structured support over several weeks or months.

You will not be managed by a single provider working alone. The team coordinates. If something is not working, we adjust.

Need a letter for work, school, or sport?

Many patients need documentation to return to work, school, or sport. Let us know what you need on your registration questionnaire. We include a return-to-work or return-to-school plan in the rehab specialist portion of your first visit. Other forms and letters carry a fee and are not covered by OHIP or private insurance.

Managing your care online

Our patient portal gives you access to appointment details, documents, and communication with your care team. it allows you to book your follow up appointments with our care team.

The platform is Input Health. Once you are registered, all clinical communication comes through the portal. You can view messages from your care team, check upcoming appointments, and request follow-up visits.

Our policy applies to all services, including OHIP-covered physician visits. Fees are not eligible for reimbursement through private insurance.
